New/Existing Patients

When treating mental health, there is no “one size fits all” method to treatment. At New Moon Health & Wellness, we individualize our client’s mental and emotional needs. Treatment will involve a combination of different treatment methods, therapy and medication management if necessary. We strongly encourage ALL patients to connect with us by using the patient portal, and following us on Facebook and Instagram for new products & services and important updates.

General Information

  • Be sure to bring your insurance cards and expect to pay your copay at the time of visit.
  • Initial appointments are scheduled for 45 minutes.
  • If the patient is a minor (under 18), both the child and parent/guardian must be present for the entire initial appointment.
  • The patient must bring medical records and medications/medication bottles to all appointments
  • Medications are prescribed at appointments and should last between appointments. If not, send us a message on the portal. There will be no early refills on controlled substances.
  • Please call at least 24 hours prior if you need to cancel or re-schedule an appointment
  • Please note that if you arrive more than 15 minutes late to your appointment, you may be asked to re-schedule
  • The office will be closed to observe all federal holidays
  • In case of inclement weather, closings will be announced on the main page of our website, and on our social media accounts Facebook & Instagram.

Self Pay Patients

The cost of self-pay services for individuals are as follows:

$100.00 for the initial appointment
$55.00 for each follow-up visit
